Überlegungen zum agora-klaustrophoben Dilemma des Borderline-Patienten

Borderline-Patienten präsentieren klinisch oft spezifische Ängste, die neurotischen oder psychotischen Ängsten ähneln können, aber in mancherlei Hinsicht von ihnen verschieden sind. Eine solche Angstkonstellation wurde von Henry Rey als agora-klaustrophobes Dilemma beschrieben, in dem der Patient weder die Nähe noch die Getrenntheit von seinen Objekten ertragen kann. In der vorliegenden Arbeit wird diese Angstkonstellation als Folge von Schwierigkeiten beim Aufbau des psychischen Raumes beschrieben, die dazu führen, dass Gefühle des Eingeschlossenseins mit Gefühlen der Verlassenheit und Leere in rascher Reihenfolge abwechseln. Anhand klinischer Vignetten werden die zugrundeliegenden Vorgänge illustriert, die häufig mit einer eingeschränkten Fähigkeit zur Symbolisierung und Wiedergutmachung einhergehen. Klinisch zeigen sich diese Phänomene in charakteristischen Übertragungs-/Gegenübertragungskonstellationen, die diagnostisch wegweisend sein können und zugleich erhebliche behandlungstechnische Probleme aufwerfen.

Summary:

Some thoughts on the agora-claustrophobic dilemma in borderline patients. – Clinically, borderline patients frequently present specific anxieties that may appear similar to neurotic or psychotic anxieties but in fact differ from these in a variety of ways. Henry Rey termed this kind of anxiety constellation »agora-claustrophobic dilemma«, the dilemma being that patients find both proximity to and separation from their objects equally difficult to bear. The present article describes this constellation as a consequence of difficulties in building up a psychic space, resulting in feelings of claustrophobic constriction alternating in quick succession with feelings of emptiness and abandonment. The author draws upon case vignettes to illustrate the underlying processes, which are frequently associated with a reduced capacity for symbolization and reparation. Clinically, these phenomena reveal themselves in characteristic transference/countertransference constellations that are reliable diagnostic indicators but at the same time pose considerable treatment problems of a technical nature.

Résumé:

Réflexions à propos du dilemme agora-claustrophobe du patient borderline. – Les patients borderline présentent souvent cliniquement des angoisses spécifiques qui peuvent ressembler à des angoisses névrotiques ou psychotiques mais en diffèrent de plusieurs manières. Henri Rey a décrit cette constellation d’angoisses en tant que dilemme agora-claustrophobe dans lequel le patient ne peut supporter ni la proximité ni la séparation de ses objets. L’auteur de cette étude décrit cette constellation d’angoisse en tant que conséquence de difficultés à construire l’espace psychique lesquelles conduisent à l’alternance rapide de sentiments d’être enfermé et de sentiments d’abandon. Des cas cliniques illustrent les processus fondamentaux accompagnés souvent d’une capacité restreinte à la symbolisation et à la réparation. Ces phénomènes se révèlent cliniquement dans des constellations caractéristiques de transfert/contre-transfert et peuvent à la fois faciliter le diagnostic et soulever d’importants problèmes de technique de traitement.
