Zur Handhabung der Vertraulichkeit bei der Veröffentlichung von Behandlungsberichten

Der Beitrag möchte eine Handreichung bereitstellen, wie Autoren mit dem Problem der Vertraulichkeit bei der Veröffentlichung von Behandlungsberichten umgehen können. Dabei wurden der gegenwärtige Diskussionsstand und die Stellungnahmen von Fachleuten eingearbeitet. Ausgangspunkt ist der Konflikt zwischen dem fachlichen Publikationsinteresse einerseits und dem persönlichen Schutzinteresse der Patienten und der analytischen Beziehung andererseits. Neben der juristischen Dimension wird diejenige der Psychoanalyse und ihrer Ethik diskutiert. Die Vor- und Nachteile eines psychoanalytisch qualifizierten »informed consent« werden angesprochen und die Möglichkeiten einer Anonymisierung aufgezeigt, die in jedem Fall erforderlich ist. Leitend ist dabei die Auffassung einer Prozessethik, in deren Entscheidungsprozesse nicht nur die einzelnen Analytiker, sondern auch ihre Patienten und die Verantwortungsgemeinschaft der Psychoanalytiker als ethische Akteure eingebunden sind.

The article is a guide to the ways in which authors can handle the problem of confidentiality in treatment reports for publication. It reflects recent discussion on the issue and the recommendations advanced by experts. The starting point is the conflict between the motivation for publication and the patients’ personal and legitimate interest in protecting both themselves and their analytic relations. The author discusses not only the legal dimension but also the ethical implications of psycho­analysis. In so doing, he addresses the advantages and drawbacks of psychoanalytically qualified informed consent as well as the various ways of assuring anonymization, which is of course indispensable in each and every case. Central to the argument is the vision of an ethical process involving in its decision-making not only individual analysts but also the psychoanalytic profession as a community of actors fully aware of their ethical responsibilities.

Cet article vise à fournir des conseils aux auteurs confrontés au problème de la confidentialité dans la publication des comptes rendus de thérapies. L’état actuel des discussions et les avis des experts ont été pris en compte. Le point de départ est le conflit entre l’intérêt que présente une publication d’un point de vue professionnel et la nécessité de protéger les intérêts des patients et la relation analytique. Outre la dimension juridique, celle de la psychanalyse et de son éthique est abordée. Les avantages et les inconvénients d’un « consentement éclairé » qualifié de psychanalytique sont abordés ainsi que les possibilités d’anonymisation, indispensable dans tous les cas. L’idée directrice est la nécessité d’une éthique du processus qui implique dans les processus de décision non seulement les analystes pris individuellement, mais aussi leurs patients et la communauté responsable des psychanalystes en tant qu’acteurs éthiques.

Sigmund Freud über traumatisch verursachte Psychosen

Ausgehend von empirischen Befunden und theoretischen Konzepten der Gegenwart stellt die vorliegende Untersuchung die Frage nach den historischen Wurzeln einer Theorie traumatisch verursachter Psychosen in Freuds Schriften. Den Hintergrund bildet der alte Vorwurf, Freud habe zugunsten der Entwicklung seiner Trieblehre reale Grenzverletzungen ungenügend berücksichtigt. Für nicht-psychotische Störungen wurde dies inzwischen eindeutig widerlegt. Der Beitrag zeigt auf, dass Freud auch für Psychosen über die verschiedenen Phasen seiner Theoriebildung hinweg traumatische Erlebnisse ätiologisch berücksichtigte. Als missverständlich und aus heutiger Sicht problematisch erscheint jedoch, dass er weniger komplexen Erwägungen seiner Schüler, etwa Karl Abrahams, öffentliche Zustimmung zollte.

Proceeding from empirical findings and contemporary theoretical concepts, the author inquires into the historical roots of a theory of trauma-induced psychoses in Freud’s works. The background is the hoary old accusation that Freud neglected real boundary violations in favor of the development of his drive theory. While this accusation has already been effectively refuted in connection with non-psychotic disorders, the present article demonstrates that throughout the various phases of theory formation Freud did indeed take account of psychoses as well. From a present-day perspective, however, one circumstance that gives rise to potential misunderstandings and problems is the fact that Freud voiced public agreement with some of the over-simplified reflections advanced by his followers, e.g. Karl Abraham.

À partir des résultats empiriques et des concepts théoriques actuels, la présente étude pose la question des racines historiques, dans les écrits de Freud, d’une théorie des psychoses causées par un traumatisme. En arrière-plan se trouve la vieille accusation selon laquelle Freud n’aurait pas suffisamment tenu compte des violations réelles des limites dans le développement de sa théorie des pulsions, ce qui a entre-temps été clairement réfuté pour les troubles non psychotiques. L’article montre que Freud a également pris en compte les expériences traumatiques dans l’étiologie des psychoses au cours des différentes phases de développement de sa théorie. Aujourd’hui toutefois, son approbation publique de considérations moins complexes de ses élèves, Karl Abraham par exemple, semble problématique et porter à malentendu.

»Now Moments« im therapeutischen Dialog
Ein Einblick in die Praxis der szenisch-narrativen Mikroanalyse (SNMA)

Der Beitrag stellt die exemplarische Untersuchung eines Psychotherapiegesprächs mit Hilfe der »szenisch-narrativen Mikroanalyse« (SNMA) vor. »Jetzt-Momente« (»Now Moments«) und »Begegnungsmomente« (»Moments of Meeting«) im Sinne von Daniel Stern und der Boston Change Process Study Group wurden von unabhängigen geschulten Ratern identifiziert und in einer systematischen, psychoanalytisch interpretierenden Konsentierung evaluiert. Ziel des Verfahrens ist es, durch die Nutzung von Parallelprozessen die Übertragungs-Gegenübertragungs-Bewegung im Therapiedialog zu verdeutlichen und dadurch ein wesentliches Agens in (nicht nur) psychoanalytischen Psychotherapien nachzuweisen.

The paper presents an example of scrutinizing a psychotherapy session by means of »Scenic Narrative Microanalysis« (SNMA). Trained raters identified independently »Now Moments« and »Moments of Meeting« as defined by Daniel Stern and the Boston Change Process Study Group. Moments were then evaluated in a psychoanalytically interpreting consensus session. The method aims at using parallel processes to describe the developing transference-countertransference relatedness in therapeutic dialogues, thus showing an essential agent in (not only) psychoanalytic psycho­therapy.

. – Cet article propose une approche théorique d’un entretien psychothérapeutique conduit à l’aide de la « microanalyse scénique-narrative » (MASN). Les « Now Moments » et les « Moments de rencontre » au sens de Daniel Stern et du Boston Change Process Study Group ont été identifiés par des évaluateurs indépendants formés à cet effet et évalués dans le cadre d’un consensus systématique, interprétatif et psychanalytique. L’objectif de cette méthode est de clarifier le mouvement du transfert et du contre-transfert dans le dialogue thérapeutique par l’utilisation de processus parallèles et de mettre au jour un agent essentiel à l’œuvre dans les psychothérapies, psychanalytiques ou non.
