Zusammenfassung:

Gerontopsychiatrische Patienten haben oft traumatisierende Kriegserfahrungen. Jahrzehnte später können erstmals belastende Erinnerungen im Gedächtnis wieder auftauchen. Im Osten Deutschlands waren Gewaltanwendungen am Ende des II. Weltkriegs häufiger als im Westen. Neben einer verzögert auftretenden posttraumatischen Belastungsstörung (Post-traumatic Stress Disorder PTSD) wird bei diesen Patienten oft eine depressive Komorbidität diagnostiziert. Stellt diese eine Zweiterkrankung dar oder ist sie Teil der PTSD im Sinne einer komplexen PTSD? Der besondere Fall: In Folge einer diagnostischen Koloskopie traten bei einer bislang psychisch gesunden 89-jährigen Patientin erstmals Albträume und Intrusionen auf. Anamnestisch waren vielfache Vergewaltigungen durch sowjetische Militärangehörige zu eruieren. Eine schwere wahnhafte Depression mit Schuld-, Versündigungs- und nihilistischem Wahn maskierte eine PTSD. Zusammenfassung: Zeitgeschichtliches Denken bei der Anamneseerhebung ist erforderlich. Bei sexuell traumatisierten gerontopsychiatrischen Patienten ist die Notwendigkeit invasiver genitaler und analer Untersuchungen kritisch abzuwägen und eine interdisziplinäre geriatrische und gerontopsychiatrische Zusammenarbeit unabdingbar. Die diagnostische Klassifikation der verzögert auftretenden PTSD nach ICD-10 ist problematisch, da sie nur eingeschränkt auf gerontopsychiatrische Patienten anwendbar ist. Depressive Komorbidität sollte extra verschlüsselt werden.

Summary:

Many psychogeriatic patients are marked by traumatic war experiences. Stressful memories may surface for the first time decades after the event. In the eastern part of Germany acts of violence occurred more frequently at the end of World War II than in the west of the country. Alongside delayed-onset PTSD (posttraumatic stress disorder) these patients are frequently diagnosed with depressive comorbidity. Is this a disorder in its own right, or is it part of a complex instance of PTSD? The case: Subsequent to diagnostic colonoscopy, an 89-year-old patient apparently in the best of mental health was suddenly affected by nightmares and intrusion. Anamnesis revealed that she had been raped on various occasions by members of the Soviet armed forces. PTSD was masked by severe delusional depression with feelings of guilt, sin and nihilistic delusion. Discussion: The investigation of a patient’s medical history invariably calls for consideration of the relevant biographical and social context. In the case of sexually traumatised psychogeriatric patients, the necessity for invasive genital and anal explorations needs to be given close critical scrutiny, and interdisciplinary geriatric and psychogeriatric cooperation is indispensable. Diagnostic classification of delayed-onset PTSD in accordance with ICD is problematic because of the restrictions besetting the classification of psychogeriatric patients. Depressive comorbidity should be encoded separately.
