Detailed report on the psychoanalytic treatment of a 37 years old patient who had been suffering for 13 years from constant stomach troubles and a recurrent ulcer duodeni. Besides he showed a great number of compulsory symptoms, among them speaking and writing inhibitions and partial impotence. The central factor of the patient's psychodynamics proved to be the conflicting experiences at the oral stage of development. In that phase the patient's emotionality became fixated and his personality development came to a standstill, so that from then on his life lost the historical dimension. The patient's concepts of eternity were significantly conditioned by his fixation on the oral stage and the repression of his ambivalent affects. He wished to be connected with Heaven by a funnel and thus be nourished. With reference to a paper of G. Bally, this aspect of orality, namely to remain in a time-less communication with a nutritive heaven, received particular consideration. The patient had achieved, without apparent difficulties, the various steps of social adjustment ( profession, marriage, family ). According to a pseudo-male ideal of activity he repressed the prevailing oral ambivalence whence originated ever recurrent similar conflicts. Moreover, the patient developed a compulsor behaviour and spoke of being a »dead bundle of papers«. In 311 sessions, analysis brought about a relaxation of the patient's compulsions and finally the removal of the oral fixation. The patient was able to relive the ambivalence of his oral-receptive and oral-aggressive tendencies and came to realize them as the sources of his lifelong sense of guilt and self-punishment. By the end-phase of his analysis, the partial drives which had been distorted in the repression, were essentially integrated and the personality of the patient changed. For the first time in many years, the stomach troubles vanished and the patient could eat whatever food he liked.

An einem lange analytisch-psychotherapeutisch behandelten Fall einer schweren, seit der Kindheit bestehenden Zwangsneurose wird die Entstehung der zwangsneurotischen Charakterstruktur und der Zwangssymptome als abhängig von spezifischen Umweltfaktoren geschildert. Es zeigte sich, daß durch die Behandlung dieser sich ständig verschlechternden und auch nicht andeutungsweise phasisch verlaufenden Zwangsneurose die Zwänge vollständig zu beseitigen waren, was als Beleg für die prinzipielle Heilbarkeit auch derartig schwerer Zwangsneurosen anzusehen ist. Die Behandlungserfahrungen legen eine Modifikation der ursprünglichen psychoanalytischen Methode nahe. Die methodische Abwandlung hat sich bereits bei anderen Behandlungen von Zwangsneurosen bewährt. Es war während der Behandlung kaum zu vermeiden, daß die ein Leben lang unterdrückten, übersteuerten dynamischen Kräfte in einer Übergangsphase in unter- und ungesteuerten affektiven Durchbrüchen und starken Willkürtendenzen zum Vorschein kamen. Es wird die Frage diskutiert, ob nicht die Psychotherapie der Zwangsneurosen bisher als so schwierig galt, weil eine übergangsweise Annahme solcher Durchbruchsphasen nicht versucht wurde. Für die Akzeptierung einer derartigen Übergangsphase wird eine stationär psychotherapeutische Klinikbehandlung empfohlen. Abschließend werden einige Ergebnisse klinischer Beobachtungen an 164 Patienten mit zwangsneurotischer Charakterstruktur mitgeteilt, von denen 20 ausgesprochene Zwangssymptome, die 144 übrigen Patienten vorwiegend körperliche Symptome hatten. Unter den Organsymptomen wird eine häufig beobachtete Symptomkopplung (Kopfschmerzen, Obstipation, Schlafstörung, funktionelle Herzstörungen) als »zwangsneurotisches Organsyndrom« bezeichnet. Das Zustandekommen der Zwangsstruktur vollzog sich im Prinzip ähnlich wie bei dem dargestellten Fall (vgl. Tabelle). Als neues Ergebnis der klinischen Beobachtungen wird über Zusammenhänge zwischen zwangsneurotischer Charakterstruktur und Funktionsstörungen des extrapyramidalen Systems berichtet. Es wird angenommen, daß durch die frühzeitige Hemmung der motorisch-aggressiven Bedürfnisse auch die zentralnervöse Regulation der Motorik über Großhirnrinde und Stammganglien des extrapyramidalen Systems gestört wird. Die regelmäßig bei Parkinsonschen Erkrankungen gefundene zwangsneurotische Charakterstruktur läßt die Frage noch offen, ob auch primär-organische Störungen des extrapyramidalen Systems die gesunde motorische Entfaltung der Persönlichkeit und die dazugehörige Erlebnisverarbeitung erschweren und auf diesem Wege die Enstehung einer zwangsneurotischen Charakterstruktur begünstigen können.

In a long psycho-analytical treatment of a case of obsessional neurosis, the development of the patient's character structure and of her symptoms was recognized as dependent from specific milieu factors. The obsession which had a deteriorating tendency without showing signs of a phasic progress, was eventually solved. Experiences made in the course of the treatment suggest a modification of the strict analytical proceeding, a method which has been tested, in the meantime, in various other cases of obsession. In one phase of the treatment, the dynamic forces which during the patient's lifetime had been over-controlled, flared up in uncontrolled affective eruptions and a wilful obstinate behaviour. The author wonders whether psychotherapy of obsessional neuroses has up to now been judged too difficult but for the reason that therapists did not wish to risk similar eruptive phases. Such phases may call for a period of clinical psychotherapy. Observations made on 164 patients with obsessional character structures showed that only 20 suffered from manifest obsessional symptoms, while 144 had mainly bodily complaints. A frequently observed combination of symptoms (e. g. headache, obstipation, disorder of sleep and functional heart disturbances) is described as a typical »obsessional organic syndrom«. The character structures of the 164 patients had developed, generally speaking, on similar lines as that in the case reported in extenso. Clinical observations further allow to assume some sort of relation between obsession and certain functional disturbances of the extrapyramidal system. The author suggests that early inhibitions of the locomotive aggressive impulses may succeed in disturbing the central nervous regulation of motion via cerebral cortex and basal ganglions. On the other hand, the obsessional character structure which is regularly found with patients suffering from Parkinson's disease, gives rise to the still unsolved question whether also primary organic disturbances of the extrapyramidal system may result in obstructing the normal motoric expressiveness of the personality with its specific modes of reacting, and thus promote development of an obsessional character.
