Auf der Grundlage von Erfahrungen einer Kommission zur Bearbeitung ethischer Probleme aus psychoanalytischen Behandlungen wird die Frage ethischer Kategorien für psychoanalytisches Handeln erörtert. Die Tätigkeit des Psychoanalytikers als Handeln im besonderen Feld der psychoanalytischen Situation enthält die Strukturen des allgemeinen Denkens über Ethik und muß gleichzeitig den Besonderheiten des psychoanalytischen Erfahrungsraums Rechnung tragen. Kodifizierungen ethischer Kategorien für psychoanalytisches Handeln sind vor allem als Aussagen über sittliche Übereinstimmungen im Rahmen der Tugendethik (Kant) zu betrachten. Erörtert wird die Frage einer möglichen Abgrenzung zum judikativen Bereich. Psychoanalytische Institutionen ihrerseits benötigen angemessene Strukturen für die Aufgaben zur Wahrung ethischer Belange bei der Anwendung der psychoanalytischen Methode. Dargestellt wird die Konzeptualisierung einer Ethikkommission, die versucht, auch in ihren normativen Aspekten psychoanalytisches Denken zu erhalten. Klinische Erfahrungen veranschaulichen die Überlegungen.

Psychoanalytic action, the psychoanalytic situation, and the question of ethics
The article discusses the question of ethical categories for psychoanalytic activity on the basis of the experience gathered by a commission dealing with ethical problems resulting from psychoanalytic forms of treatment. Seen in terms of the specific sphere represented by the psychoanalytic situation, the activity of psychoanalysts encompasses the structures of general thinking on ethics and must at the same time take account of the specific features of the situation they are operating in. Attempts to codify ethical categories for psychoanalytic activity must be regarded in the first place as statements about moral imperatives in the framework of an »ethics of virtue« (Kant). The article then goes on to discuss potential demarcations over and against the judicative sphere. Psychoanalytic institutions, for their part, require structures that will enable them to address ethical concerns in the application of psychoanalytic methods. The author enlarges on the conceptualization of an ethics commission dealing with issues that include the preservation of the normative aspects of psychoanalytic thinking. Clinical experience is drawn upon to illustrate the train of thought set out in the article.

L’action du psychanalyste, la situation psychanalytique et la question de l’éthique
En se fondant sur les expériences d’une commission pour traiter les problèmes éthiques, rencontrés dans les thérapies psychanalytiques, l’auteur discute la question des catégories éthiques pour l’action psychanalytique. L’activité du psychanalyste en tant qu’action dans le champ particulier de la situation psychanalytique comporte les structures de la pensée générale de l’éthique et, en même temps, elle doit prendre en compte les particularités de l’espace expérimental psychanalytique. Il faut considérer les codifications des catégories éthiques pour l’action psychanalytique surtout comme des énoncé sur la conformité avec la morale dans le cadre de l’éthique de la vertu (Kant). L’auteur discute la question d’une délimitation possible de ce domaine du domaine judiciaire. De leur côté, les institutions psychanalytiques ont besoin de structures adéquates pour accomplir la tâche de respecter les intérêts éthiques lors de l’application de la méthode psychanalytique. L’article présente la conceptualisation d’une commission d’éthique qui, même dans ses aspects normatifs, cherche à conserver la pensée psychanalytique. Les réflexions sont illustrées par des expériences cliniques.

Nach einer Bestimmung der Merkmale schwerer Hypochondrie wird die regressive Dynamik und der Rückzug auf das phantasmatische Leben skizziert, die in einer destabilisierenden Reintrojektion münden. Die Reintrojektion bedroht das seelische System. Mit ihr entfaltet sich ein pathogener Rückzug, in dem sich eine Dynamik zwischen Introjekt, Organ und geschwächtem Ich etabliert, die gespeist wird aus frühkindlich-psychotischen, traumatischen Quellen. Gesteuert wird dieser Rückzug über Fixierungen, die intensiv besetzte narzißtisch oder traumatische Selbstanteile sind, in denen die Erfahrungen mit dem mütterlichen Objekt eingewoben sind. Psychogenetisch entwickelte sich eine psychogen autistische Organisation, in denen das Objekt nicht mehr als container gedacht werden kann und projektive Identifizierungen versagen. Die Mitteilung der Verstehen ermöglichenden Affekte erfolgt häufig im Schutze dissoziative Zustände, die eine unbewußte Entäußerung von Auflösungszuständen ermöglicht. Die behandlungstechnischen Konsequenzen solcher autistoiden Übertragung und deren Überwindung werden diskutiert. Zwei detaillierte klinische Vignetten illustrieren diese Dynamiken.

Autistoid withdrawal dynamics in severe hypochondria
The article begins with a definition of the features of severe hypochondria, proceeding from there to outline the regressive dynamic and the withdrawal to fantasmatic life involved in this condition and culminating in a destabilizing reintrojection that poses a threat to the psychic system. This reintrojection goes hand in hand with pathogenic withdrawal in which a dynamic operating between introject, organ, and weakened ego establishes itself. This dynamic is powered by infant-psychotic, traumatic sources and is organized via fixations representing intensively cathected narcissistic or traumatic parts of the self in which experiences with the maternal object are intimately involved. In psychogenetic terms, there develops a psychogenetically autistic species of organization, in which the object can no longer be thought of as a container and projective identifications fail to function. The communication of affects facilitating understanding frequently takes place under the protective mantle of dissociative states that are made possible by the unconscious alienation of states of dissolution. The article continues with a discussion of the treatment-related consequences of such autistoid transference and of ways in which it can be overcome. Two detailed clinical vignettes illustrate the dynamics described.

Dynamique autistique du replie en cas d’hypochondrie grave
Après avoir défini les symptômes de l’hypochondrie grave, l’auteur esquisse la dynamique régressive et le replie sur la vie fantasmatiques, qui débouchent sur une réintrojection déstabilisante. La réintrojection menace le système psychique. Avec elle, s’effectue un replie pathogène où s’installe une dynamique entre l’introjet, l’organe et le Moi affaibli, qui se nourrit des sources psychotiques, traumatiques de la petite enfance. Ce replie est régulé par des fixations qui sont des parties du soi, investies narcissiquement de manière intense, ou traumatiques, dans lesquelles sont entremêlées les expériences avec l’objet maternel. Sur le plan psychogénétique, une organisation autistique psychogène s’est développée où l’objet ne peut plus être conçu comme container et où les identifications projectives échouent. La communication des affects permettant la compréhension se fait souvent à l’abri d’états dissociatifs, ce qui permet une extériorisation inconsciente des états de dissolution. Les conséquences pour la technique thérapeutique d’un tel transfert autistique ainsi que son dépassement sont discutées. Deux présentations détaillées de cas cliniques illustrent ces dynamiques.
