Der Artikel verortet institutionelle psychodynamische Beratung in Form der Ehe-, Familien-, Lebens- und Erziehungsberatung im Kontext benachbarter Disziplinen, insbesondere der Psychotherapie. Die Unterscheidung der Formate Psychotherapie und Beratung vomVerfahren psychodynamische Therapie ist dabei besonders hilfreich. Es wird ein summarischer Überblick über das bestehende, zumeist kirchlich getragene institutionelle Beratungsangebot gegeben. Das in diesem Bereich dominante psychodynamische Ausbildungskonzept, ein exemplarisches Ausbildungsmodul aus der OPD und eine Fallvignette werden beschrieben. Hinweise auf Evaluationsstudien und zur psychosozialen Nachhaltigkeit von institutioneller psychodynamischer Beratung schließen den Artikel ab.

Institutional psychodynamic counselling – a format of psychosocial care
The article intends to give institutional psychodynamic counselling a place in the realm of psychosocial, medical and especially psychotherapeutic care. The author stresses the difference between »format« and »technique«. He labels »psychotherapy« and »counselling« two different »formats« and considers »psychodynamic therapy« a »technique« applicable in both formats. Descriptions of the landscape of an institutional primarily church related counselling program in Germany and of one predominant concept of training in psychodynamic counselling are given followed by a brief report on a training unit and a case description. Finally the article mentions some ideas on the psychosocial sustainability of psychodynamic counselling and refers to studies reporting empirical evidence.

Die Lebens- und Problemlagen von Ratsuchenden in der Ehe-, Familien-, Lebens- und Erziehungsberatung (EFLE-Beratung) werden immer unübersichtlicher und komplexer. Berater sind hier in ihrer diagnostischen Kompetenz herausgefordert, um ihrem Auftrag – »Hausarzt« für psychosoziale Fragen zu sein – gerecht werden zu können. Mit dem hier vorgelegten Schema einer szenischen Diagnostik wird dafür plädiert, im diagnostischen Prozess somatische, biografische, soziometrische (beziehungsdynamische), gesellschaftliche und axiologische (normative und existenzielle) Befunde zu berücksichtigen. Darüber hinaus ist es aus berufsethischen Gründen unerlässlich, die erkenntnistheoretischen Grenzen der Diagnostik deutlich zu benennen.

Scenic understanding in marriage, family, life and child guidance counselling – a diagnostic map for a field of high complexity
The situation in life and the problems of people that seek advice from marriage, family, life and child guidance counselling become more and more confusing and complex. The counsellors are asked to develop a special diagnostic competence, to cope with their assignment of being a »family doctor« for psychological and social questions. We discuss a pattern of scenic diagnosis, to mark the place of physical, biographical, sociometrical (material dealing with the dynamic of relationships), social and axiological (normative and existential) material within each diagnostic process. Furthermore, for professional ethical reasons it is necessary, to name clearly the epistemological limitations of diagnostics.

Der Artikel untersucht, ob ein psychodynamisch reflektierter Umgang mit Beratungsgeschehen möglich ist. Das Praxisfeld beider Autorinnen, die Psychologische Beratung an Hochschulen, bildet den Bezugsrahmen. Es wird gezeigt, inwieweit psychodynamische Prozesse, wie sie aus der psychotherapeutischen Arbeit bekannt sind – das Herstellen eines Arbeitsbündnisses, die Arbeit mit der Beziehung, die Funktion des Containings, der »Moment der Begegnung«, Konflikte, Übertragung und Gegenübertragung sowie Deutungen – auch in der Beratungskommunikation vorkommen. Als Reflexionskategorien für das Verstehen von Beratungsinteraktion und für professionelles Beratungshandeln scheinen diese Konzepte unverzichtbar.

Interactions and psychodynamics in counseling
The article explores the possibility of psychodynamically reflected counselling. Student counselling is the frame of reference of both authors. It is shown how concepts from psychotherapy – like work-alliance, use of the therapeutic relation, containing, »moment of meeting«, conflicts, transference, counter-transference and interpretation – work in counselling. These concepts are indispensable for both understanding counselling-interactions and guiding professional activities.

Die Behandlungsmethode der »Transference-focused Psychotherapy« (TFP) entwickelte Otto F. Kernberg in den letzten 20 Jahren auf der Grundlage seiner Konzeption der Borderline-Persönlichkeitsorganisation.Bei dieser psychodynamischen Psychotherapie der Borderline-Persönlichkeit handelt sich um eine modifizierte, störungsorientierte Form der analytischen Psychotherapie, die zweistündig im Sitzen stattfindet, von einem Therapievertrag gestützt wird und zentral an der Übertragungsbeziehung im Hier-und-Jetzt arbeitet. Primäre Therapieziele sind die Reduzierung der Borderline-spezifischen Symptome wie z.B. Depression, Angst und Suizidalität, selbstschädigendes Verhalten, unzureichende Kontrolle über Affekte, impulsivesVerhalten und Agieren sowie eine Verminderung der Gefahr von Therapieabbrüchen. Strukturelles Ziel der Behandlung ist es, fragmentierte innere Bilder vom Selbst und den anderen zu integrieren, die Reflektionsfähigkeit und die interpersonellen Beziehungen zu verbessern. Das TFP-Therapiemanual liegt seit 2001 deutschsprachig in manualisierter Form vor und gibt klare und differenzierte Anweisungen zu Diagnostik, Therapievertrag und Behandlungsstrategien. TFP-Fortbildungs-Curricula werden vom deutschen TFP-Institut in München und an anderen Orten im deutschen Sprachraum angeboten. Erste empirische Untersuchungen konnten die Wirksamkeit der TFP bei Patienten mit Borderline-Persönlichkeitsstörungen belegen.

Transference-focused psychotherapy (TFP)
Transference-focused Psychotherapy (TFP) has been developed by Otto F. Kernberg during the past 20 years and is based on his theoretical concepts of borderline personality organisation. This psychodynamic psychotherapy for borderline personality represents a modification of psychoanalysis that takes place twice a week while sitting, is supported by a treatment contract, and focuses on the transference and countertransference in the here-and-now. The aims of the treatment are reduction of borderline specific symptoms, e.g. depression, anxiety, suicidality and self-mutilation, deficient affect control, impulsiveness, acting-out and breaking up therapy. On a structural level the aim of TFP is to integrate fragmented inner representations of the self and significant others and to improve reflective functioning and quality of object relations. In 2001 a German translation of the TFP manual was published. The manual gives detailed instructions on diagnostic procedures, contract setting and treatment strategies. TFP training courses are delivered by the German TFP institute in Munich and in other German speaking places. Preliminary results from TFP intervention studies support the effectiveness of the treatment.
