Die Verbesserung der Versorgung schizophrener Störungen hat für die Betroffenen aber auch gesundheits-ökonomisch und volkswirtschaftlich hohe Relevanz. Neben einer individuell zugeschnittenen möglichst wirksamen und verträglichen antipsychotischen Pharmakotherapie haben die letzten Jahre eine zunehmende Anzahl von als wirksam belegten störungs- und problemspezifischen verhaltenstherapeutischen Behandlungsansätzen hervorgebracht. Sie zielen dabei auf eine Verbesserung der Behandlungsvoraussetzungen wie Förderung von Behandlungsbereitschaft und nicht-resignative Krankheitsakzeptanz sowie von therapeutischer Beziehungsqualität, Verbesserung der sozialen Kognition und Neurokognition, von sozialer Unterstützung und Reintegration am Arbeitsplatz sowie Umgang mit Antipsychotika-resistenter Restsymptomatik.

Multimodal therapy of persistant voices and antipsychotic resistent delusions: evidence and future directions
Development and delivery of evidence-based treatment for schizophrenia is not only important for recovery of the patients but also by health economic reasons. Cognitive-behavioural interventions are tailored to illness characteristics and phase of treatment and not only problem focussed. Evidence based treatment programs are developed to address building of strong and trustful therapeutic relationship, enhancement of treatment adherence and functional adjustment to the disorder (recovery as a motivational process), improvement of social cognition and social skills as well as neurocognition respective social support by relatives or to cope with residual symptoms of the illness.

Das Rorschach-Verfahren kann wichtige Informationen zur Feststellung oder zum Ausschluss schizophrener Erkrankungen beitragen. Bei den schwach strukturierten Vorlagen manifestieren sich Veränderungen der Gestaltwahrnehmung sowie Denkstörungen. Wichtig ist, die Anwendung des Rorschach-Verfahrens auf Fragestellungen der Psychopathologie zu beschränken. Außerdem wird das Rorschach-Verfahren immer im Verbund mit anderen Untersuchungsmethoden angewendet. Valide Diagnosen beruhen immer auf der Zusammenschau der Ergebnisse unterschiedlicher Verfahren. Diese Vorgehensweise wird anhand von drei Fallbeispielen erläutert.

The Rorschach test in the diagnosis of schizophrenic psychosis
The Rorschach test is able to add some decisive information to approve or to exclude the diagnosis of schizophrenia. Changes of Gestalt Perception as well as the manifestation of thinking disorders can come to light in its poorly structured visual patterns. It is vital to restrict the use of the Rorschach test to questions of psychopathology. Anyhow, in clinical use the Rorschach test is always presented in assemblage with other tests. Valid diagnosis in general depends on the combination of different techniques. This approach is illustrated by means of three case studies.

Die überwiegend positiven Erfahrungen telepsychiatrischer Untersuchungen der letzten 20 Jahre und die in Deutschland bald allseits zur Verfügung stehende optimierte Geräte- und Netzwerktechnik für Videokonferenzen sollte nun politische und rechtliche Veränderungen einleiten, die einer Online-Psychiatrie und Psychotherapie Qualitätsstandards und Rechtssicherheit geben. SMS-, Chat-, Email- und Videokontakte werden heute in zahlreichen Projekten und Studien psychiatrisch-psychotherapeutisch eingesetzt und im Ausland vereinzelt bereits regulär genutzt. Wir sollten uns fachlich vermehrt in die Digitalisierung der Kommunikation einbringen und uns nicht durch Verbote und unerfüllbare technische, rechtliche und finanzielle Auflagen ausschließen lassen.

Telepsychiatric consultation
The in majority positive experiences of telepsychiatric consultations/studies of the past 20 years and the availability of optimised hardware and networking for videoconferencing at lower costs now should introduce political and legal changes that grant »online-psychiatry« quality standards and legal security. SMS-, chat-, email- and videocontacts are nowadays in scattered use in numerous projects and studies. Therefore we should put more of our efforts into digitalisation of communication channels not to be excluded by unfulfillable technical, legal or financial issues.

Der Autor fasst den aktuellen Stand der Diskussion über die theoretischen Konzepte von Phobien zusammen und vergleicht die bisher untersuchten psychodynamischen Therapieverfahren bei Sozialer Phobie mit der Katathym-Imaginativen Psychotherapie. Die aktuellen Störungskonzepte nehmen strukturelle und konfliktbedingte Ursachen an, was auch bei der Planung tiefenpsychologisch fundierter Therapie zu berücksichtigen ist. Im Therapiemodell von Leichsenring et al. (2007b) fußt die Diagnostik und Behandlung auf dem Zentralen Beziehungskonflikt-Thema und der Supportiv Expressiven Therapie (SET) nach Luborsky, für die Anwendung der KIP hingegen schlägt der Autor die Diagnostik und Behandlungsplanung anhand des OPD-2 vor. Die Darstellung der KIP-Behandlung mit sowohl Struktur- als auch Konflikt-orientierten Interventionen wird illustriert anhand von 2 Fallbeispielen, letztlich ist aber die systematisch empirische Evaluation unumgänglich.

Social phobia: treatment with the guided affective imagery Therapy
The author presents current theorization of phobias and discusses the psychodynamic therapy for social phobia with respect to the guided affective imagery therapy. Present concepts of anxiety disorders assume dispositions in psychic structure and conflicts as of etiologic relevance. This has to be considered when psychodynamic therapy is planned and undertaken. The treatment manual of Leichsenring et al. (2007b) concerning conflict diagnosis and therapy is based on Luborsky ´s Core Conflictual Relationship Theme (CCRT) and the Supportive Expressive Therapy (SET), for the guided affective imagery treatment though the author suggests using the Operationalized Psychodynamic Diagnostics (OPD-2) System for treatment planning. Guided affective imagery therapy thus includes structureas well as conflict-based interventions and is presented here with 2 case reports. This approach has yet to be empirically evaluated.
