Psychotherapeutisches Gedankengut lässt sich auf Grund eines ganzheitlichen Menschenbildes bis ins Altertum zurückverfolgen. Ärztliche Psychotherapie entwickelte sich im 19. und 20. Jahrhundert sowohl aus philosophischer Richtung als auch in suggestiv-übenden Verfahren. Diese Entwicklungslinien gingen von der Vorstellung aus, dass seelische Störungen körperliche Ursachen haben. Auch der Begründer der Psychoanalyse Sigmund Freud war dieser Ansicht. Erst in der Ausdifferenzierung der psychoanalytischen Schulen einerseits und der Weiterentwicklung suggestiver Verfahren aus der Hypnose andererseits, entwickelte sich die ärztliche Psychotherapie seit der ersten Hälfte des letzten Jahrhunderts. Erst später kam die Verhaltenstherapie hinzu, welche sich auf der Grundlage von Lerntheorien auf das Umlernen pathologischen Verhaltens richtete. Die verschiedenen Therapieprinzipien bestehen heute nebeneinander. Die Besonderheit der ärztlichen Psychotherapie liegt in ihrer vorwiegend psychodynamischen bzw. anthropologischen Ausrichtung, in der Differenzialdiagnostik und dem mehrdimensionalen Prinzip sowie der Beachtung psychosomatischer als auch somatopsychischer Störungsbilder. Die Tradition der ärztlichen Psychotherapie setzt sich fort sowohl in der Kompetenz des Facharztes für psychosomatische Medizin und Psychotherapie und für Psychiatrie und Psychotherapie, als auch in der Integration in weitere Fachgebiete im Rahmen der fachgebundenen Psychotherapie.

Medical psychotherapy in Germany, yesterday – today – tomorrow
Psychotherapeutic ideas concerning a holistic view of man existed already in the early history. Medical psychotherapy evolved in the 19th and 20 th century both from philosophy and from suggestive-relaxating methods. These lines of development emerged from the hypothesis of psychic disturbances to be of somatic origin. Even the founder of psychoanalysis Sigmund Freud had this opinion. By the differentiation of psychoanalytic schools and the development of suggestive approaches evolved the medical psychotherapy in the first half of the 20th century. In Germany the behavioural therapy joined later. These principles of therapy are applied in Germany. The specifity of medical psychotherapy bases mainly on psychodynamic- anthropologic orientation, on the medical »differentialdiagnosis« and on the so called »multidimensional principle« as well as on the regarding psychosomatic and somatopsychic disturbances. The tradition of medical psychotherapy in Germany continues in the competence of the »Facharzt für Psychosomatik und Psychotherapie« (Physician of Psychosomatic and Psychotherapy) and is moreover integrated in other medical disciplines.

Die medizinische Versorgungslandschaft der psychischen Erkrankungen hat sich in der Bundesrepublik Deutschland in den letzten 20 Jahren u.a. durch die Einführung des Facharztes für Psychosomatische Medizin oder das Psychotherapeutengesetz erheblich gewandelt. Aber auch durch neue wissenschaftliche Erkenntnisse und die Entwicklung neuer psychotherapeutischer Methoden verändern und erweitern sich die Möglichkeiten psychotherapeutischer Arbeit. Im Spannungsfeld zwischen Methodenausrichtung einerseits und Berufspolitik andererseits ist es deshalb sinnvoll und notwendig, zukunftsfähige Modelle für den jetzigen Facharzt für Psychosomatische Medizin und Psychotherapie zu erarbeiten. Entsprechende Ideen und Vorschläge der DÄVT (Deutsche Ärztliche Gesellschaft für Verhaltenstherapie) dazu werden vorgestellt und erörtert.

Creating relationships and identity in medical psychotherapy – musings of a behavioural therapist
The medical background for the treatment of psychological disorders has changed considerably in Germany over the last 20 years, inter alia due to the introduction of the title »Specialist for Psychosomatic Medicine« and the Psychotherapist Act. But the opportunities provided by psychotherapeutic work have also been changed and expanded by new scientific findings and the development of new psychotherapeutic methods. In the field of tension between methodological approach on the one hand, and professional policies on the other, it is therefore both appropriate and necessary to prepare future-oriented models for today's specialist for psychosomatic medicine and psychotherapy. The author introduces and discusses corresponding ideas and suggestions from the DÄVT (German Society for Medical Behaviour Therapy).
