Schwangerschaft und Postpartalzeit gelten als typische Schwellensituationen im Leben einer Frau und eines Paares und fordern Adaptationsleistungen im somatischen und psychosozialen Bereich. Psychische Störungen in dieser Lebensphase zeichnen sich durch ihre unmittelbaren Auswirkungen auf das (un)geborene Kind und Besonderheiten in der psychotherapeutischen Behandlung aus. Im folgenden Beitrag (Teil I) wird ein überblick über die Epidemiologie sowie phänomenologische Besonderheiten psychischer Störungen im Peripartum gegeben. Weiterhin werden die transgenerationalen Auswirkungen mütterlicher psychischer Störungen in der Peripartalzeit auf den weiteren Schwangerschaftsverlauf, die Geburt, die Mutter- Kind-Bindung und die kindliche Entwicklung dargestellt (Teil II des Beitrags in diesem Heft gibt einen überblick über Behandlungsoptionen).
Mental disorders during pregnancy and postpartum, part I
Pregnancy and the postpartum period are considered as typical life events for a woman and the respective couple. They require major somatic and psychosocial adaptations. Mental disorders during this period of life have a relevant impact on the (un)born child and require specific interventions. The following paper (part I) gives an overview about the epidemiology and phenomenological characteristics of mental disorders related to maternity. Additionally the current knowledge about the trans-generational importance of these problems for the course of pregnancy, delivery, the mother-child-relationship and the infant development is summarized. (Part II offers an overview about effective interventions.)
Psychische Störungen in der Schwangerschaft und Postpartalzeit stellen einen Risikofaktor für den weiteren Schwangerschaftsverlauf, die Geburt, die kindliche Entwicklung, die Mutter- Kind-Bindung und letztendlich nachfolgende Schwangerschaften dar. Spezifische niedrigschwellige psychotherapeutische und psychopharmakologische Behandlungen in diesem vulnerablen Lebensabschnitt sind essenziell, um Schaden von Mutter und Kind abzuwenden. Durch die Besonderheiten der Peripartalzeit ergibt sich hier eine große Herausforderung für alle beteiligten Behandler. Der Beitrag gibt einen überblick über wirksame Behandlungsansätze sowie die Möglichkeiten und speziellen Anforderungen einer interdisziplinären Netzwerkarbeit.

Mental disorders during pregnancy and postpartum represent a risk factor for the course of pregnancy, birth, infant development, mother-child-bonding and finally future pregnancies. Specific low-threshold psychotherapeutic and psychopharmacological interventions are essential to prevent mother and child from harm during this vulnerable period of life. The particularities of the peripartal time are challenging for all professionals who are involved in the treatment. This paper gives an overview about effective interventions as well as options and special demands for interdisciplinary networking.
Etwa 40% der Patienten, die für eine Lebertransplantation evaluiert werden, weisen eine Störung durch Alkohol auf. In der Beurteilung ihrer Eignung für eine Lebertransplantation sollten neben den allgemeinen Faktoren des psychosozialen Funktionierens wie beispielsweise psychische Vorerkrankungen, Gesundheitsverhalten und Compliance auch suchtspezifische Prädiktoren eines Rezidivs untersucht werden. Durch eine gruppentherapeutische Behandlung kann die Abstinenz verbessert und auf diese Weise die Wartezeit verlängert werden. äthylglukuronid als ein innovativer Alkoholmarker ermöglicht eine größere Transparenz in der Beurteilung und Behandlung des Suchtverhaltens. Nach der Transplantation ist bei ca. 15% der Patienten ein Rückfall in den Alkoholkonsum zu erwarten. Annähernd 5% der Patienten haben im Rahmen der Lebertransplantation, insbesondere der Diagnosemitteilung, posttraumatische Belastungen entwickelt. Diese Daten machen deutlich, dass es sich bei den Lebertransplantationspatienten mit äthyltoxischer Leberzirrhose um eine Gruppe mit großem psychosomatischen Behandlungsbedarf handelt. Durch psychosomatisch-psychotherapeutische Interventionen können der Behandlungserfolg, die körperliche und die psychische Lebensqualität der Patienten verbessert werden.
Patients with alcohol related disorders in the frame of liver transplantation
About 40% of the patients who are evaluated for a liver transplantation exhibit an alcohol related disorder. In the assessment of their eligibility for a liver transplantation addictionspecific predictors for a relapse should be also examined beside the general factors of psychosocial functioning like prior psychiatric disorders in history, health behaviour and compliance. Abstinence can be improved by a group-therapeutic treatment and therefore the waiting period can be extended. Ethylglucuronide as an innovative alcohol marker allows a major transparency in the assessment and treatment of addiction behaviour. After the transplantation a relapse is to be expected in approximately 15% of the patients. Nearly 5% of the patients have developed post-traumatic disorders within the frame of the liver transplantation, in particular in relation to the diagnosis communication. These data make clear that liver transplant patients with ethyltoxic cirrhosis are in distinctive need of psychosomatic treatment. By psychosomatic-psychotherapeutic interventions the success of treatment, physical and mental quality of life of the patients can be improved.
Psychotherapie sowie komplementär-/alternativmedizinische Behandlungsmethoden (CAM) machen es Patientinnen mit Krebs einfacher, sich auf individuelle Art und Weise mit der Krankheit auseinandersetzen und darauf Einfluss nehmen zu können sowie die Lebensqualität zu verbessern. Selbsthilfegruppen spielen sowohl in der psychosozialen Versorgung krebskranker Frauen als auch in der Vermittlung von Informationen über Behandlungsmethoden eine wesentliche Rolle. Aus diesem Grund haben wir Leiterinnen von Selbsthilfegruppen für krebskranke Frauen (n=342) nach deren Einstellungen gegenüber Psychotherapie und Komplementär- bzw. Alternativmedizin befragt. Wir kamen zu dem Ergebnis, dass Leiterinnen von Selbsthilfegruppen für krebskranke Frauen gegenüber psychotherapeutischen und komplementär- bzw. alternativmedizinischen Fragen recht offen sind. ältere Personen und Personen mit einem niedrigeren Schulabschluss scheinen mit diesen Fragen jedoch überfordert zu sein. Gerade bei diesen Frauen sollten psychotherapeutische und komplementär- bzw. alternativmedizinische Fragen in Fortbildungsveranstaltungen verstärkt thematisiert werden. Künftige Forschung sollte nicht nur die Einstellungen untersuchen, sondern auch das Wissen sowie den Umgang mit psychologischen und psychotherapeutischen Aspekten einer Krebserkrankung.
Attitudes towards psychotherapy, complementary and alternative medicine of leaders of self help groups for female patients with cancer – a review including an empirical study
For female cancer patients psychotherapy as well as complementary/alternative methods of treatment (CAM) facilitate an individual approach to their illness, a possible influence on the therapy process, and improve quality of life. Support groups play a decisive role in the psychosocial care of women suffering from cancer and also in the procurement of information regarding different methods of treatment. On this account we surveyed female leaders of support groups for women with cancer (n=342) on their attitudes towards psychotherapy and complementary/alternative medicine. We came to the conclusion that female leaders of support groups for women with cancer are quite open-minded regarding psychotherapy and complementary/alternative medicine. However, older people and people with low-level education seem to be overstrained with such issues. Especially with these women issues around alternative treatment approaches and psychotherapy should be addressed more strongly in further education. Future research should not only investigate attitudes towards but also knowledge about and dealing with psychological and psychotherapeutic aspects of cancer.
Die Psychoonkologie stellt eine Herausforderung in vielerlei Hinsicht dar: eine noch junge Fachdisziplin mit unklaren Definitionen, Qualifikationen und Finanzierungen, die sich Menschen in existenziellen Krisensituationen widmet. Krisen finden sich in vielen Lebensbereichen, doch scheint die Diagnose einer Krebserkrankung neben den erheblichen psychosozialen Belastungen bei allen Beteiligten einen häufig nicht objektiv begründeten Schrecken auszulösen. So sind das Entsetzen und der Diagnoseschock ungleich größer als bei Menschen, denen die Diagnose einer unheilbaren neurologischen oder kardiologischen Erkrankung mit ebenso schlechter Prognose mitgeteilt wird. Dass Krebserkrankungen in vielen Fällen auch heilbar sein können, ist zunächst gar nicht im spontanen Gedankenrepertoire engrammiert. An die psychotherapeutischen Behandler und Begleiter werden ebenfalls besondere Herausforderungen gestellt. So ist der Psychoonkologe bei den Betroffenen nicht immer willkommen, sein Setting muss sich den körperlichen Kapazitäten der Patienten anpassen, Ausfalltermine gehören zur Routine und Zuspitzungen im Krisenerleben erfordern Notfalltermine in nicht planbarer Zahl. Schlussendlich erfordert die psychoonkologische Arbeit eine höchst reflektierte Auseinandersetzung mit der eigenen Existenz und den eigenen Widerständen.
The challenge of psycho-oncology
Psycho-oncology poses a challenge in different manners: it´s a rather new discipline with unclear definitions, qualifications and financing dealing with people in existential crises. Crises are not unusual in health care, but the perception of cancer is totally different as utterly devastating in all cases. Beyond all psychosocial distress the trauma getting a diagnosis of cancer crushes down much more than any other incurable disease as cardiopathy or neuropathy with a comparable or worse prognosis. During the shock of diagnosis there is no awareness for cancer diseases could be cured. The psychotherapists are especially challenged as well. Psychooncologists are not really welcome at any time. The setting has to be as flexible as even possible: considering the physical capacities of patients, drop out dates and emergency dates which make it impossible to schedule the work. Finally psycho-oncology demands a strong self reflection about the own mortality and the own resistances facing the finitude.
In dem Beitrag werden die psychoonkologischen Betreuungsaspekte in einem Brustzentrum exemplarisch dargestellt. Mit der Zentrumsbildung zur Behandlung brustkrebserkrankter Frauen sind die psychoonkologischen Aspekte der Betreuung integraler Bestandteil der Therapie geworden und die praxisorientierte Umsetzung für Patientinnen mit primärem Mammakarzinom wird dargestellt.

Die Diagnose Krebs erschüttert, lässt das Leben eines betroffenen Menschen aus den Fugen geraten und verändert es auf ungewollte und ungewünschte Weise. Die persönliche Konfrontation mit der Diagnose stellt eine einschneidende Erfahrung dar, die von Betroffenen fast immer als unbarmherzige Zäsur im eigenen Lebensrhythmus erlebt wird. Diesem Artikel liegen zum einen Erfahrungen zugrunde, die die Autorin selbst als an Krebs Erkrankte erlebt hat, und zum anderen fließen Erlebnisse von betroffenen Frauen ein, die sie in ihrer Funktion als Landesvorsitzende der Frauenselbsthilfe nach Krebs, Rheinland-Pfalz/Saarland e.V. im Selbsthilfealltag kennengelernt und erfahren hat. Die gewonnenen Einsichten werden verdichtet und aus der persönlichen Perspektive einer Betroffenen zum Ausdruck gebracht. Behandelnde ärzte, Therapeuten, Pflegende, Seelsorger etc. – sind tagtäglich konfrontiert mit Patientinnen und Patienten, die ähnliches fühlen, ähnliches erleben und ähnliche Gedanken haben. Und doch ist es ihnen aus unterschiedlichsten Gründen – Zeitmangel, eigene Unsicherheiten und ängste, angepasstes Verhalten der Patientinnen, Wort- und Sprachlosigkeit auf beiden Seiten – oftmals nicht möglich die Innensicht ihrer Patientinnen so zu erfassen, wie es ihnen gelingen könnte, wenn die betroffenen Frauen dazu den notwendigen Raum, die Zeit und eben auch die Ermutigung zur »Entäußerung« ihrer Gefühle und Gedanken bekämen. Der hier ins Wort und Bild gebrachte »persönliche Erfahrungsbericht« soll »Behandlern« einen kleinen Einblick in die Innenwelt eines Menschen ermöglichen, dem die Diagnose Krebs mitgeteilt wird. Dadurch wird sichtbar gemacht, welche Unterstützungsangebote Betroffene – besonders in kommunikativer Hinsicht – benötigen und als hilfreich erleben.
Diagnosis cancer
The diagnosis cancer shocks, lets the affected person´s life fall apart, and changes it in an unmeant and unwished way. Almost always the personal confrontation with the diagnosis cancer is a drastic encounter, experienced by affected people as a cruel break in the rhythm of life: a deep cut between the past and future. This article is based on the author´s experiences facing her own cancer as well as those of affected women she daily came to know during her position as regional chairperson of the »women´s self-help after cancer« organization. She will combine the gained insights into the story of an affected person. Those who care for patients with cancer - doctors, therapists, care takers, clergy etc. – are confronted daily by patients who have similar experiences, similar feelings and similar thoughts. However, due to very different reasons – lack of time, their own insecurities and fears, adjusted behaviour of the patients, loss of words and speechlessness on both sides – it is often not possible for them to grasp the introspection of the patient. This could be accomplished if the affected women were provided with the necessary space and time, as well as the encouragement to »let go« their emotions and thoughts. These described experiences put into words and pictures, would allow the treatment provider a small glimpse into the inner life of a person dealing with the diagnosis of cancer. This will bring to light the particular needs of the patient, and the necessary and helpful support required, particularly regarding communication.
Die psychotherapeutische Behandlung schwerer Persönlichkeitsstörungen und psychosomatischer Akutzustände ist häufig mit dem Problem der Affektüberflutung und impulsiver Fehlhandlungen verbunden, welche das therapeutische Setting oftmals sprengen und die psychotherapeutische Behandlung zum Scheitern bringen können. In dieser Behandlungssituation kann eine pharmakotherapeutische Intervention passager stabilisierend wirken und einen Therapieabbruch verhindern. Das Ziel der vorliegenden Arbeit ist die Operationalisierung des Medikamenteneinsatzes nach Maßgabe der psychodynamischen Einordnung unterschiedlicher Impulsabwehrkonstellationen. Diese Herangehensweise bleibt daher nicht auf eine additive Pharmakotherapie begrenzt, sondern integriert die Pharmakotherapie in den psychotherapeutischen Behandlungsprozess. Hieraus ergibt sich daher ebenfalls eine Handlungsanleitung für das Ausschleichen oder Absetzen der Medikation.
Psychodynamics of the interaction of psychotherapy and pharmacotherapy
The objective of this paper is the integration of medication in the psychotherapeutic treatment of patients with personality disorder and acute psychosomatic symptoms. Since these patients are often overwhelmed by affect-aden impulses, following either an inner crisis or a crisis within the transference relationship, the pharmacological treatment with antipsychotic drugs is a valid option in order to hold onto the setting and sustain sychotherapy. In the present paper we suggest that pharmacological intervention needs to be planned in accordance with the organization of the defense mechanisms within the psychodynamic structure of the individual patient. The paper presents guidelines not only for the implementation of pharmacological treatment but also for the discontinuation of medication, for example if the therapeutic process slows down due to a lack of motivation on the part of the patient when his symptoms have become less pernicious following drug treatment. The preconditions for the successful treatment of these patients are discussed.
