Auf eine Phase lebhaften psychoanalytischen Interesses an den psychosomatischen Krankheiten ist inzwischen eine Phase der Skepsis gefolgt. Seit etwa 1960 hat eine Gruppe von französischen Autoren (David, Fain, Marty, de M'Uzan) – unter Umgehung der ätiologischen Problematik – eine spezifische Persönlichkeitsstruktur der psychosomatisch Kranken postuliert. Cremerius stellt die These auf, daß diese »psychosomatische Struktur« nicht krankheitsspezifisch, daß sie vor allem nichts »Primäres« ist. Ausgehend von eigenen Erfahrungen, die er durch Fallskizzen illustriert, macht er geltend, daß die von den französischen Autoren getroffene Patienten-Selektion und die von ihnen arrangierte Untersuchungssituation das Erscheinungsbild der »psychosomatischen Struktur« in hohem Maße determinieren.

Is the »psychosomatic structure« of the French school syndrome-specific?
Following a period of lively psychoanalytic interest in psychosomatic illnesses, a phase of skepticism has taken over. Beginning about 1960 a group of French authors (David, Fain, Marty, de M'Uzan) has postulated a specific personality structure for the psychosomatically ill without consideration of the problern of etiology. Cremerius asserts that this »psychosomatic structure« is not syndromespecific and that it is not »primary«. On the basis of his own experience, illustrated with case outlines, he argues that the French authors' patient-selection and the examination situation arranged by them largely determine the phenomenon of the »psychosomatic structure«.

Beschrieben wird ein bestimmter Typus von psychosomatisch Kranken, der zuerst durch die Unfähigkeit zum freien Phantasieren auffiel und dessen Eigenart (neutralisierte Beziehungen zum Therapeuten wie zu den Personen der Umwelt – »relation blanche«; schematisches Denken; definitiver Sieg des Realitätsprinzips, traumloser Konkretismus) die Gruppe um de M'Uzan dann mit dem Terminus »operatives Denken« zu kennzeichnen suchte. Das »operative Denken« geht häufig dem Auftreten somatischer Störungen voraus, kündigt sie an.

On the psyochology of the psychosomatically ill
The author describes a certain type of psychosomatic patient who is conspicuous because of his incapacity for free fantasy. M’Uzan and his coworkers have designated the peculiarities of this type with the term »operative thinking«, namely, a neutralized relation- ship to the therapist as well as other people in the environment (»relation blanche«), schematic thinking, definitive victory of the reality principle, dreamless concretism. »Operative thinking« frequently precedes and heralds the appearance of somatic disturbances.

Overbeck argumentiert methodenkritisch und phänomenorientiert gegen die Identifizierung psychosomatischer Krankheiten mit der »pensée opératoire« bzw. der »Alexithymie« – also mit patho-physiologischen, sinnlosen Reaktionen auf der Basis einer defekten Ich-Organisation. Im Anschluß an K. Brede und andere Autoren vertritt er die Gegenthese, daß gerade die psychosomatischen Symptome Produkte sinnhafter und origineller Ich-Leistungen (im Medium einer differenzierten Körper- und Organsprache) sind.

Psychic deficiency or generative ego achievement?
Addressing himself to methodological and phenomenological-substantive issues, Overbeck argues against the tendency to identify psychosomatic illnesses with the »pensée opératoire« or »alexithymia«, that is, with physiopathological, meaningless reactions occurring on the basis of defective ego organization. Rather, he argues in agreement with K. Brede and other authors, advocating the counter-thesis that psychosomatic symptoms are the products of meaningful and original ego achievements in the medium of a differentiated body and organ language.

In der Perspektive einer noch zu erarbeitenden einheitlichen Theorie der psychosomatischen Leiden erscheint das psychosomatische Symptom – das Resultat einer Konfliktverarbeitung auf dem somatischen Ausdrucksfeld – nicht nur (neurosenpsychologisch) als Funktions-Äquivalent der Abwehr, sondern (sozialpsychologisch) zugleich als Vermeidung sozialer Devianz durch Flucht in die sozial akzeptierte Krankenrolle.

A social-psychological approach to the specificity of psychosomatic disturbances
In the perspective of an integrative theory of psychosomatic illnesses, the somatic symptom resulting from the resolution of a conflict within the somatic field of expression would appear not merely as the functional equivalent of a defense (theory of neurosis) but also (social-psychologically) as the avoidance of deviant behavior insofar as symptom formation involves the adoption of the sick role and the regression which is admissible within this behavior pattern.
