Foucaults Haltung zu Freud und zur Psychoanalyse hat von seinen frühesten Werken, zumal Wahnsinn und Gesellschaft, bis zu seinen letzten, Sexualität und Wahrheit, fortwährend geschwankt: Dem frühen Lob Freuds, eine kritische Gegenwissenschaft entwickelt und dem Wahnsinn wieder eine Stimme gegeben zu haben, steht am Ende die Verdammung der Psychoanalyse als normierender Wissenschaft im Rahmen des Sexualitätsdispositivs gegenüber. Der Autor geht dem wechselvollen Schicksal dieser Herausforderung, die Freud offensichtlich für Foucault darstellte, nach. Er kommt dabei zu grundlegenden Einsichten gleichermaßen in die Wirkmächtigkeit Freuds wie in tiefreichende Motive des Werks und der Person von Michel Foucault. Dazu gehören sowohl die unaufgelöste Spannung im Verhältnis zur Theorie und Praxis der »Überschreitung« als auch der prekäre Status der Selbstreflexion.

Freud, Foucault and the »Dialogue with Unreason«
From his early works (notably Madness and Society) to his late History of Sexuality, Foucault’s attitude to Freud and psychoanalysis was highly ambivalent. The early plaudits for Freud’s creation of a critical, anti-establishment theory that gave insanity a voice to speak with are offset by the later pillorying of psychoanalysis as a normative science in the framework of the dispositif of sexuality. The author traces the various implications of the challenge Freud obviously represented for Foucault and arrives at far-reaching conclusions both about the immense impact exercised by Freud and the profounder motives underlying Foucault’s own work and character. Among these are the unresolved tensions between the theory and practice of »transgression« and the precarious status of self-reflection.

Obwohl die Metapher die klinische Arbeit und Theorie der Psychoanalyse in vielfacher Weise dominiert, wird sie selbst nur selten zum Gegenstand der Analyse und bleibt ihre Rolle weitgehend unbemerkt. Unter Hinweis auf die kognitive Linguistik analysiert der Autor die Metapher als wichtiges Moment der Behandlungspraxis. Er unterscheidet zwischen manifesten und konzeptuellen Metaphern und zeigt, daß die psychoanalytische Deutungsarbeit als Metaphernwechsel konzipierbar ist, das heißt, daß die Deutung eine neue Metapher ins Spiel bringt und damit auch neue Handlungsräume erschlossen werden können. Anhand mehrerer klinischer Fallbeispiele expliziert der Autor seine Hypothesen auf anschauliche Weise.

Metaphor and Psychoanalytic Dialogue
Although the clinical theory and practice of psychoanalysis is in many ways dominated by metaphor, this fact has been rarely subjected to systematic analysis and the role it plays has hence gone largely unremarked. Taking his bearings from cognitive linguistics, the author discusses metaphor as an important element in psychoanalytic treatment. Distinguishing between manifest and conceptual metaphors, he demonstrates that psychoanalytic interpretation may be conceptualized in terms of »metaphor switching«. The interpretation brings a new metaphor into play, thus opening up new vistas and new avenues to be explored. With reference to a number of case examples, the author is able to provide a graphic illustration of the implications of his hypothesis.

Trotz der Bemühungen um eine Integration von klassischer Medizin und Psychosomatik wird das Gesundheitswesen im wesentlichen von der klassischen Medizin getragen. Anhand eines Modells bzw. einer Metapher, der »Trivial-Maschine« und der »Nicht-Trivial-Maschine«, versucht Buchinger das Dilemma zwischen klassischer und psychosomatischer Medizin zu erläutern. Mit Hilfe dieses Modells gelingt es ihm, die These zu untermauern, der zufolge nicht die psychosomatische Medizin in die klinische Medizin zu integrieren sei, sondern die psychosomatische Medizin die allgemeineren Grundlagen jeglicher Behandlung von Krankheit bereithalte (womit jedoch nicht gemeint ist, daß umgekehrt die klinische Medizin real in die psychosomatische Medizin integrierbar sei). Buchinger spricht von der »Expertise des Nichtwissens«, die der psychosomatische Mediziner vertrete. Als Experte des Nicht-Wissens könne er den Beziehungsraum erweitern und dem Patienten helfen, sein je eigenes »Wissen« hervorzubringen und auf diese Weise zu lernen, sich »gesünder« zu bewegen.

Why Psychosomatics is still an Also-Ran. System constraints in medicine
Notwithstanding all the efforts undertaken to integrate classical medicine with psychosomatics, the health system is still squarely based on »classical« medical science. With reference to the model (or metaphor) of the »Trivial Machine« versus the »Non-Trivial Machine«, Buchinger attempts to cast light on the dilemma bedeveling the relation between classical and psychosomatic medicine. With the help of this model he is able to substantiate his thesis that the aim should not be to integrate psychosomatic medicine into clinical medicine but that psychosomatic medicine has the broader foundations for any kind of treatment of illnes to build on (which does not mean that vice versa clinical medicine could be integrated into psychosomatic medicine). According to Buchinger, the rationale behind this is the »expertise of not-knowing« which (he suggests) the psychosomatic doctor represents. As an expert in »not-knowing«, the psychosomaticist can push back the frontiers bounding the relational space between doctor and patient and help the patient to bring forth and capitalize on his/her own »knowledge« and thus learn to behave in a more »healthy« way.
